Watershed Blood Supply
Tissue fed by a single terminal or retrograde vessel has no redundancy, so injury there heals badly.
Watershed blood supply describes tissue perfused at the far end of one arterial territory, or by a single vessel running in an unhelpful direction, with no collateral route to take over if that vessel is compromised. The term borrows from drainage basins: a watershed is the thin boundary where supply from either side is weakest. Anatomy of this kind removes redundancy, and the clinical consequence is that an ordinary insult produces an extraordinary outcome.
The scaphoid is the canonical example. Most of its blood arrives through dorsal branches entering distally and running retrograde toward the proximal pole. A fracture across the waist can therefore cut off the proximal fragment entirely, which is why proximal pole fractures carry far worse prognosis than distal ones — the same fracture pattern, moved a centimetre, becomes a different disease. Comparable arrangements exist in the femoral head, the talus, the odontoid process, and the splenic flexure of the colon.
When perfusion fails outright the result is avascular necrosis: bone dies, loses structural integrity, and collapses under load, often months after the injury and after apparent healing. Dead bone cannot participate in Bone Remodeling, so it is neither replaced nor repaired on its own, and the risk of Fracture Nonunion rises sharply.
This anatomy drives clinical caution. Scaphoid fractures are notoriously invisible on initial radiographs, a plain Sensitivity and Specificity problem, so a suggestive examination is often immobilised and re-imaged rather than cleared. It also justifies early Fracture Fixation in fragments at risk, and it explains why perfusion governs outcomes in soft tissue too, as in Moist Wound Healing.
