Thermoregulation is the set of physiological mechanisms that hold core body temperature within a narrow range regardless of the surrounding environment. The hypothalamus acts as the controller, comparing measured blood temperature against an internal set point and triggering corrective responses on either side of it — the same architecture as a thermostat, and one reason the analogy is used so often, though the biological version has no single fixed setting and shifts under different conditions.

Below the set point, the body conserves and generates heat: blood vessels near the skin constrict (vasoconstriction), redirecting blood away from the surface to reduce heat loss to the environment, and skeletal muscle begins involuntary rapid contraction — shivering — which generates heat as a byproduct of muscular work with no actual movement intended. Above the set point, the reverse mechanisms engage: vasodilation brings more blood to the skin surface to radiate heat away (the flush of overheating), and sweat glands secrete fluid whose evaporation carries heat off the skin, which is why humidity matters as much as temperature — sweat that can't evaporate cools nothing.

Fever is not thermoregulation failing; it is thermoregulation working normally against a deliberately raised set point. Pyrogens released during infection — part of Inflammation's signaling cascade — instruct the hypothalamus to defend a higher temperature, which is why a feverish person feels cold and shivers even while their measured temperature climbs — the body is chasing a set point that has moved, not losing control of one that hasn't. This is also why fever breaks with sweating: once the pyrogen signal clears, the set point drops back down and the body suddenly needs to shed the excess heat it had been actively maintaining.
